I have also been informed of my right to refuse to contribute without reprisal, and my contribution has not been made under actual or threatened physical force, job discrimination, financial reprisals, or as any condition of employment

I am an American Citizen or have been admitted to permanent U.S. residence and this contribution is made in my name. I understand that contributions to Charter Texas PAC are not deductible as a charitable contribution on Federal Income Tax returns. All payroll deductions are effective immediately.

I understand that any payroll deduction will remain in effect until revoked or changed by me and that I may revoke or change my payroll deduction via my written communication to the PAC administrator; a member of the Charter Government Affairs Department.
